This waiver is required to be signed by the parent/guardian for each EDHSC player participating in training sessions or any EDHSC activities.

Assumption of the Risk and Waiver of Liability to Coronavirus/COVID 19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to be spread mainly from person to person contact. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic.

I understand the hazards of the novel coronavirus (“COVID-19”) and am familiar with the Centers for Disease Control and Prevention (“CDC”), State of California, County and City guidelines regarding COVID-19. I acknowledge and understand that that the circumstances regarding COVID-19 are changing from day to day and that, accordingly, the CDC guidelines are regularly modified and updated and I accept full responsibility for familiarizing myself with the most recent updates.  I further understand that COVID-19 is extremely contagious and is believed to be spread mainly from person to person contact. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic. I also understand that the risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease;


El Dorado Hills Soccer Club places the safety of our children as it’s number one priority. EDHSC has implemented preventative measures to reduce the spread of COVID-19. However, the Club cannot guarantee that you or your children will not be infected with COVID-19. Further, attending and participating in our activities could possibly increase your risk of contracting COVID-19.

By signing this agreement, I ACKNOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK on behalf of myself and my child(ren) that I or my child(ren) may be exposed to or infected by COVID-19 by participating in an El Dorado Hills Soccer Club program(s), related event, or activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself, my spouse, my child(ren), guests, unborn child or relatives. I understand the risk of becoming exposed or infected by COVID-19 at an EDHSC activity may result from the actions, omissions, or negligence of myself, and others, including but not limited to EDHSC employees, board members, volunteers, program participants and their families. I also acknowledge that I have reviewed the “Return to Play” policy implemented by EDHSC and I understand and agree to adhere to all guidelines.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance of EDHSC activities or participation in the Club’s programming (“Claims”).

By allowing my child(ren)to participate in any of the Activities, I understand and represent that at all times that:

* No one in my immediate family (“Family”) is experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* No one in my Family has traveled internationally within the last 14 days.

* No one in my Family has traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe anyone in my Family have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* No one in my Family has been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.

* My Family is following all CDC recommended guidelines as much as possible and limiting my interactions with others.

In addition to the above, I willingly agree that my child(ren) and myself will comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my or my child(ren)’s presence or participation, including any of the above issues, I will remove my child(ren) and or myself from participation and bring such to the attention of the nearest official and coach immediately; and on my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless EDHSC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of EDHSC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during or after participation in any EDHSC program.
